Acute skin refractory graft-versus-host disease, after allogeneic hematopoietic stem cell transplant

This is the case of a 23-year-old woman with B-cell acute lymphocytic leukemia, who underwent an allogeneic transplant of hematopoietic stem cells from an unrelated HLA-identical donor, complicated by cutaneous acute graft-versus-host disease, without involvement of other organs, refractory to corticosteroids. In the first instance tacrolimus was used as a chronic immunosuppressant, which was later replaced with sirolimus. High doses of steroids were also used. Mycophenolate mofetil and gamma globulins were used without response. With the use of ruxolitinib and infliximab gradual improvement was achieved. Currently the patient is without signs of active graft-versus-host disease.
