Primary cutaneous recurrent marginal zone B-cell lymphoma. Case report and literature review

Primary cutaneous B-cell lymphomas account for approximately 20-25% of all primary cutaneous lymphomas. Primary cutaneous marginal zone B-cell lymphoma (PCMZL) is the second most frequent primary cutaneous B cell lymphoma. It is designated as a separate entity by the World Health Organization classification as part of the marginal zone lymphoma; this was previously included in extranodal marginal zone lymphoma of mucosa-associated lymphoid tissue. It has an indolent course and an excellent prognosis. We present the case of a 49-year-old female patient, who consulted for a localized subcutaneous tumor of 1 cm of diameter on the left forearm. An excisional biopsy was performed which showed a nodular infiltrate in the depth dermis composed of mononuclear cells, some with scarse cytoplasm and others with large clear cytoplasm. Plasma cells were seen in the periphery of the infiltrate. The immunohistochemistry of neoplastic cells showed CD20+, BCL-2+, CD43+ y CD10−, plasma cells were CD138+, and lambda and kappa chains were positive, with predominance of the first ones. This patient had previously PCMZL, so recurrence of this disease was diagnosed. The recurrence of primary cutaneous marginal zone B-cell lymphoma is frequently seen. PCMZL is a low-grade lymphoma with an overall 5-year survival of 95-99%.
